All this effort is great! So worth it, and something we all struggle with. I had to do Lantus twice a day before pumping. Adjusting your I:C ratio is also good. Takes loads of documentation. You can do it though!!

You know, you could try this square bolus technique that we pumpers have the luxury of using when we bolus. It's supposed to be used for "fatty foods" but I use it for breads and pastas as well. You might experiment with taking say 2/3 of your H dose and then the other 1/3 a half hour or an hour after eating and see if that doesn't help those numbers come down without shooting straight up right after eating. Another thing some people do is wait 20-30 minutes after dosing before eating (I think @ElizabethArnold has mentioned this) for the insulin to really "work." I remember this technique from back in the day when I took regular insulin which took 30 minutes to work, so you always had to wait 30 minutes to eat.

Just some suggestions that you might either try or bring up with NH next time you see him. Things that I have found to be incredibly helpful as I battle my own post meal spikes.
